MI Choice Waiver Program

We administer the ‘MI Choice’ (Medicaid) Home and Community Based Waiver, a long term, in-home care program designed to provide services to the elderly and the disabled. The MI choice (Medicaid) Waiver Program provides services to help you remain independent in your own home, the home of the another, an Adult Foster Care Home, or a Home for the Aged.

Our Information and Referral Specialist can take your calls and perform an assessment in the home, hospital, or nursing home can be scheduled with one of our care coordination teams.

You may be eligible if you:
  • Aged 65 or older

  • Are 18 years of age or older and have been deemed disabled by the State or Federal Government.

  • Have income no greater than $2,829 a month, gross.

  • Have assets no greater than $2,000 for single person (special rules apply for a married couple).

  • Are a nursing home resident or would be physically eligible for nursing home placement and need support to return, or remain in, your own home or community based placement.

  • Have difficulty or are unable to perform basic activities of daily living such as personal care, bathing, and housekeeping, and/or need assistance with managing your medications.
