What is the PACE Facility?
PACE is a Program of All-Inclusive Care for the Elderly. An alternative to nursing home living, the services provided by the Sunrise PACE program will enable the elderly to age in place in their home and community.
The PACE approach
Your care is provided by an Interdisciplinary Team (IDT) that is assigned to each participant consisting of the following staff:
• Physician
• Registered Nurse
• Home Care Coordinator
• Masters level Social Worker
• Occupational Therapist
• Physical Therapist
• Recreational Therapist
• Registered Dietitian
• PACE Center Manager
• Certified Nurse Assistant
• Transportation Driver
The IDT assesses each participant’s needs and delivers services based on a customized care plan. Where needed, the plan will include acute care, hospitalization and nursing facility.

In Your Home
We deliver supplies and provide the support approved by your care team, such as:
Medications and Reminders
Prepared Meals
Medical Equipment
Caregiving and Family Support
Around the Community
We coordinate and drive you to your medical appointments, including:
Specialty Care such as Cardiology, Neurology and Ophthalmology
Scheduled Surgeries and Procedures
X-rays
When necessary, we cover emergency, hospital and long-term care services.
Who can join PACE?
Anyone who is 55 years of age or older.
Eligible for nursing home level of care as determined by the State of Michigan.
Able to live safely in the community with the support of Sunrise PACE.

How much does PACE cost?
Sunrise PACE receives payments from Medicare/Medicaid, if applicable, to cover expenses for its participants. Most people who have both Medicare and Medicaid pay nothing more for the program.
Once enrolled in PACE and Medicaid eligible, if you are admitted to a nursing facility you may be liable for a patient pay amount as determined by the State of Michigan. A PACE participant may be fully liable for the cost of medical services from an out-of-network provider or without previous authorization (except for emergency services).
