Community Health Worker – Part-time position.  Using a care transition model, work with patients being discharged or referred from hospitals or health plans to reduce the chance of re-admission. Focus is placed on coaching and empowering patients to take an active role in their post-discharge recovery and/or healthcare. Effectively educates and links patient and providers to available community-based resources by coordinating with Area Agency on Aging staff.  High school diploma or equivalent required with experience working in a community-based setting for at least 1 to 2 years preferred. Executive Assistant/Board Coordinator – Full-time position with benefits. The Executive Assistant/Board Coordinator provides high-level administrative support to company directors by handling information requests and performing clerical functions such as preparing correspondence, receiving visitors, arranging conferences, conference calls and scheduling meetings for directors.  He/she oversees and directly prepares for Board of Directors’ meetings and ensures that the meetings run smoothly.   High school diploma or equivalent required.  Associate Degree preferred, although extensive office experience may substitute for formal education.  Knowledge of office machines, equipment and office procedures inclusive of proper grammar and spelling.  Familiarity with a wide variety of the agency’s concepts, practices and procedures as well as official legal meeting procedures.  Must be able to work independently and without direct supervision and in a confidential manner.  Submit resume with cover letter to Region VII Area Agency on Aging, 1615 S. Supports Coordinator, Registered Nurse:  Region VII Area Agency on Aging is seeking a full-time RN for the MI Choice Medicaid Waiver Program.  This position is responsible for the coordination of care for elderly and adult disabled clients.  Position requires travel within the 10 county region.  Competitive salary and travel reimbursement.
